WHAT ARE THE REQUIREMENTS

Upon application for reception to the Center, the applicant shall be required to submit documents, such as but not limited to:

  1. Voluntary Confinement of Adult (18 years old * above) 

      –  Deed of Voluntary Commitment (notatized)

      –  Social Case Study Report

      –  Letter of Intent

      –   Approved endorsement from the City Mayor’s Office

      –   Photocopy of Birth Certificate

      –   Whole body picture (3R of 5R)

      –   Barangay Certificate showing his/her residency

      –   Psychiatric Evaluation Report

      –    Court Order

Components and standard cost of care and maintenance – The cost of care shall cover the total cost of direct and indirect services provided to the dependents while having his/her rehabilitation in the center.

 However, depending on the recommendation of the local social workers of the City Social Services and Development Office (CSSDFO) who will determine the financial capabilities of the dependent; the cost of care may be reduced or subsidized in the following viz:

Category A – Non-Davao Residents

For a dependent who is not a resident of Davao City, the fee is Ten Thousand Pesos Per Month (Php10,000.00/month)

 

Category B

Davao City Residents with monthly income above the poverty threshold by more that 50% the fee shall be Five Thousand Pesos Per Month (Php 5,000/month.

 

Category C

Davao City residents with monthly income above the poverty threshold by 50% or less 

For the family of a drug dependents whose monthly income is above the poverty threshold by 50% or less and can still afford to share in the expenses for the treatment, rehabilitation, food, Accommodation, and medicines, the fee shall be Two Thousand Five Hundred Pesos per month (Php2,500.00/month).

 

Category D

Davao City resident whose monthly income is below poverty threshold and declared as indigent.

For the family of a drug dependent whose monthly income is below poverty threshold and declared as indigent by the City Social Services and Development Office (CSSDO), all cost for the rehabilitation are free of charge. However, a drug dependent who is not considered indigent but to whom the CSSDO has issued a Certificate of Eligibility, all cost for the rehabilitation are also free of charge.
